FCPH offers a Public Health Nurse led Home Visiting Program

During Black Maternal Health Week and the entire year, FCPH offers a Public Health Nurse led Home Visiting Program.

We've received funding for this program since 2020 from the Franklin County Board of Commissioners, and it's in partnership with Heart of Ohio Family Health.

The Program focuses on serving pregnant/postpartum individuals of color and their families. We also build trusted relationships with clients and act as a personal care nurse/provider taking individual and family preferences into account.

We serve clients and their families by meeting with them during their scheduled prenatal/postpartum clinic appointments, in their homes or in the community per client preference.

We've seen great numbers from the program so far:

- More than 30 individuals served by the program.

- 666 touch points made with clients (includes all calls, emails, text messages, speaking with patients at clinic)

- 40 home visits/Televisits completed

- Safe Sleep education provided to all clients

We look forward to continuing to carry out this great work in our communities!
